(一)功能家庭醫(yī)生工作室是基層醫(yī)療衛(wèi)生機(jī)構(gòu)家庭醫(yī)生服務(wù)團(tuán)隊(duì)提供服務(wù)的主要場(chǎng)所。家庭醫(yī)生服務(wù)團(tuán)隊(duì)利用家庭醫(yī)生工作室為簽約居民提供個(gè)性化隱私保護(hù)的基本醫(yī)療、公共衛(wèi)生和健康管理服務(wù),如常見病、多發(fā)病、慢性病的一般診療、預(yù)約、轉(zhuǎn)診以及聯(lián)合會(huì)診或遠(yuǎn)程會(huì)診、健康評(píng)估、管理方案制定、隨訪等履約服務(wù)。

(二)基礎(chǔ)設(shè)施1.基層醫(yī)療衛(wèi)生機(jī)構(gòu)可將現(xiàn)有全科門診升級(jí)為家庭醫(yī)生工作室,或與醫(yī)聯(lián)體專家門診、聯(lián)合門診相結(jié)合設(shè)置。若設(shè)置較為集中,可設(shè)置家庭醫(yī)生簽約服務(wù)工作區(qū)。中醫(yī)藥綜合服務(wù)區(qū)的中醫(yī)診室也可增掛家庭醫(yī)生工作室,作為中醫(yī)類全科醫(yī)生提供家庭醫(yī)生簽約服務(wù)的固定場(chǎng)所。2.房屋面積不低于10平方米。放置診斷桌(至少供家庭醫(yī)生團(tuán)隊(duì)2-3人同時(shí)辦公)、就診椅、診斷床、屏風(fēng)或隔簾、上下水等設(shè)施(符合院感控制相關(guān)要求)。有條件的基層醫(yī)療衛(wèi)生機(jī)構(gòu)可配備全科一體化診療設(shè)備。中醫(yī)家庭醫(yī)生工作室還應(yīng)配備提供中醫(yī)藥服務(wù)所需的設(shè)備。3.配備電話、電腦、打印機(jī)、固定電話及居民信息采集設(shè)備、與機(jī)構(gòu)聯(lián)通的信息化系統(tǒng)。4.目前房屋面積小無法改造的機(jī)構(gòu),也可以為家庭醫(yī)生團(tuán)隊(duì)內(nèi)的護(hù)士或公共衛(wèi)生人員在診室外設(shè)置工作臺(tái),確保為簽約居民提供綜合、連續(xù)服務(wù),讓簽約居民少跑路。
(2) Infrastructure 1. (三)管理要求1.命名和標(biāo)識(shí)。統(tǒng)一命名為“XX團(tuán)隊(duì)家庭醫(yī)生工作室”,室外懸掛統(tǒng)一標(biāo)識(shí)牌(標(biāo)識(shí)牌參考樣式見附件)。如與醫(yī)聯(lián)體專家結(jié)合設(shè)置,應(yīng)加掛醫(yī)聯(lián)體坐診專家診室標(biāo)識(shí)牌。室內(nèi)在保持與健康驛站風(fēng)格一致的情況下,按照服務(wù)特色和專長進(jìn)行個(gè)性化布置。2.團(tuán)隊(duì)出診。采用團(tuán)隊(duì)出診形式提供服務(wù),通過團(tuán)隊(duì)成員之間和門診服務(wù)流程之間的有效銜接,讓居民在一次診療過程中完成其近期需接受的所有基本醫(yī)療、公共衛(wèi)生和健康管理服務(wù),突出團(tuán)隊(duì)整體性和簽約服務(wù)綜合性、連續(xù)性的特點(diǎn)。3.定向分診。家庭醫(yī)生工作室接診患者主要為簽約居民,有條件的地區(qū)可利用信息化手段采取預(yù)約、掛號(hào)自動(dòng)定向分診等方式逐步引導(dǎo)居民主動(dòng)利用簽約服務(wù)資源。應(yīng)充分保證簽約居民的就診時(shí)間(約15分鐘),診療過程中充分體現(xiàn)全方位、全周期的健康服務(wù)理念,增強(qiáng)簽約居民簽約服務(wù)獲得感。4.掛牌公示。家庭醫(yī)生工作室應(yīng)掛牌公示家庭醫(yī)生團(tuán)隊(duì)成員基本信息(姓名、職稱、聯(lián)系方式)、固定坐診時(shí)間、巡診地點(diǎn)和時(shí)間及服務(wù)監(jiān)督電話等。

(一)功能家庭醫(yī)生服務(wù)點(diǎn)是家庭醫(yī)生服務(wù)團(tuán)隊(duì)在社區(qū)、村居等為簽約居民提供巡診服務(wù)的場(chǎng)所。巡診主要為簽約居民提供健康咨詢、健康管理隨訪、慢性病患者出院或轉(zhuǎn)診情況回訪、合理用藥指導(dǎo)等以健康咨詢、健康教育、用藥指導(dǎo)和隨訪為主的簽約服務(wù),不提供具有醫(yī)療風(fēng)險(xiǎn)、非簽約服務(wù)內(nèi)容且需居民另外付費(fèi)的診療服務(wù)。
(1) Functional family doctor service points are places where family doctor service teams provide patrol services to contracted residents in communities, villages, and other areas. (三)管理要求1.統(tǒng)一命名為“XX團(tuán)隊(duì)+社區(qū)和村居名稱家庭醫(yī)生服務(wù)點(diǎn)”,室外懸掛統(tǒng)一標(biāo)識(shí)牌(標(biāo)識(shí)牌參考樣式見附件)。可設(shè)立在村衛(wèi)生室、社區(qū)衛(wèi)生服務(wù)站、社區(qū)養(yǎng)老機(jī)構(gòu)、居委會(huì)、社區(qū)綜合公共服務(wù)場(chǎng)所、社區(qū)日間照料中心等。鼓勵(lì)獨(dú)立設(shè)置用房,條件不具備的,可混合使用。2.家庭醫(yī)生服務(wù)團(tuán)隊(duì)?wèi)?yīng)制定巡診計(jì)劃,向居民公開巡診時(shí)間及安排,方便居民到社區(qū)家庭醫(yī)生服務(wù)點(diǎn)主動(dòng)接受服務(wù)。3.鄉(xiāng)、村(或中心、站)兩級(jí)共同組成家庭醫(yī)生簽約服務(wù)團(tuán)隊(duì)的基層醫(yī)療衛(wèi)生機(jī)構(gòu),鄉(xiāng)鎮(zhèn)衛(wèi)生院和社區(qū)衛(wèi)生服務(wù)中心巡診期間,要對(duì)村衛(wèi)生室、社區(qū)衛(wèi)生服務(wù)站開展的簽約居民基本醫(yī)療、公共衛(wèi)生和健康管理等服務(wù)情況進(jìn)行評(píng)估和質(zhì)量管理,重點(diǎn)為病情發(fā)生變化、控制不穩(wěn)定、依從性低、疾病復(fù)雜的簽約居民提供有針對(duì)性的綜合服務(wù)。4.有條件的地區(qū)可建立巡診預(yù)約機(jī)制,最大程度地利用好有限的巡診時(shí)間和團(tuán)隊(duì)服務(wù)資源。家庭巡診、家庭病床等家庭診療服務(wù)可結(jié)合社區(qū)巡診同步開展。5.巡診以團(tuán)隊(duì)的方式進(jìn)行,團(tuán)隊(duì)成員要穿著統(tǒng)一制服并佩戴胸牌,樹立安全防范意識(shí),并注意保護(hù)居民隱私。6.團(tuán)隊(duì)?wèi)?yīng)當(dāng)通過電子或者紙質(zhì)方式對(duì)巡診信息進(jìn)行記錄,并請(qǐng)居民通過簽字等方式進(jìn)行確認(rèn)。

The focus should be on providing targeted comprehensive services for contracted residents whose conditions have changed, control is unstable, compliance is low, and diseases are complex. 4. Regions with conditions can establish a consultation appointment mechanism to maximize the use of limited consultation time and team service resources. (一)服務(wù)功能健康驛站是基層醫(yī)療衛(wèi)生機(jī)構(gòu)設(shè)立的為居民提供自助檢測(cè)、個(gè)體化健康教育、家庭醫(yī)生簽約等服務(wù)的綜合場(chǎng)所。主要服務(wù)功能包括收集匯總居民健康信息,建立或更新居民健康檔案;提供健康教育處方和群體或個(gè)體化健康教育;開展基本公共衛(wèi)生服務(wù)項(xiàng)目和家庭醫(yī)生簽約服務(wù)政策宣傳,提供面對(duì)面簽約和常規(guī)健康管理服務(wù)。

(2) Infrastructure 1. (三)管理和工作要求1.統(tǒng)一命名為“健康驛站”,懸掛統(tǒng)一標(biāo)識(shí)牌(標(biāo)識(shí)牌參考樣式見附件),設(shè)置展板簡要介紹健康驛站服務(wù)功能。2.配備經(jīng)過培訓(xùn)的工作人員,負(fù)責(zé)自助健康檢測(cè)指導(dǎo)、政策咨詢、健康教育和其他日常管理。3.建立健康驛站管理制度、工作流程、操作規(guī)范、應(yīng)急預(yù)案等規(guī)章制度,實(shí)施規(guī)范化、制度化管理。4.健康驛站主要服務(wù)轄區(qū)內(nèi)常住居民。居民持身份證或居民檔案信息卡等有效憑證接受服務(wù)。簽約居民、慢性病患者、65歲以上老年人等重點(diǎn)人群優(yōu)先。5.健康驛站作為基層醫(yī)療衛(wèi)生機(jī)構(gòu)開展健康教育活動(dòng)的主要場(chǎng)所,制定健康教育年度和月度工作計(jì)劃,定期開展健康教育活動(dòng),宣傳“我的健康我管理”等健康理念,并在機(jī)構(gòu)顯著位置或電子屏上公開。工作日期間,每日固定時(shí)間,主動(dòng)引導(dǎo)就診居民或家屬,參與健康管理工具使用、自我健康監(jiān)測(cè)指導(dǎo)、健骨操等費(fèi)時(shí)較短、參與性強(qiáng)、簡單易學(xué)、適合候診或診后開展的小型健康教育和健康促進(jìn)活動(dòng)。6.健康驛站為居民開展自助檢測(cè),并提供或發(fā)送自助檢測(cè)結(jié)果,但不做疾病診治。對(duì)出現(xiàn)異常或陽性體征居民,建議到相應(yīng)家庭醫(yī)生工作室或門診做進(jìn)一步診斷和檢查。7.居民在家庭醫(yī)生服務(wù)團(tuán)隊(duì)成員或健康驛站配備的具有執(zhí)業(yè)資格的執(zhí)業(yè)(助理)醫(yī)師或護(hù)士的指導(dǎo)下,開展的健康檢測(cè),可作為家庭醫(yī)生簽約服務(wù)履約和基本公共衛(wèi)生服務(wù)建立或更新健康檔案、重點(diǎn)人群健康管理隨訪的依據(jù),檢測(cè)數(shù)據(jù)可以通過信息系統(tǒng)接口直接進(jìn)入居民健康檔案。無專業(yè)醫(yī)務(wù)人員指導(dǎo)下居民自助檢測(cè)所獲得的數(shù)據(jù),不能作為履約或隨訪的依據(jù)。8.建立健康驛站及與其相配套的信息系統(tǒng),有必要的信息保密及安全措施,避免檢測(cè)數(shù)據(jù)泄露。
